Who Are We?

The Ventura County Coalition for Compassionate Care (VCCCC) was established in 2012 to promote and provide education about Advance Care Planning (ACP) and Physician’s Orders for Life Sustaining Treatment (POLST) to the public and healthcare professionals. Our mandate is to educate not only the patients, but the healthcare community they collaborate with, including skilled nursing facilities, dialysis centers, hospitals and more.

VCCCC members recognize there are multiple topics that both the public and the health care community would benefit from receiving more knowledge and information about, including:

Having serious illness conversations and goals to manage the illness as it progresses

End-of-life planning possibly including POLST or hospice

Advance care planning

Funeral planning

Bereavement support

Leadership

Membership in the VCCCC includes representatives from hospitals, home health and hospice agencies, funeral homes, law firms, community-based service organizations, and any other professional who wants to do their part to advance knowledge of end-of-life planning.

“Our local coalitions have established Advance Care Planning (ACP) and POLST in their communities and continue to provide ACP and POLST education to healthcare professionals across the continuum of care, including how to use ACP and POLST properly and how to facilitate meaningful ACP and POLST conversations with patients and families. These coalitions also educate consumers about POLST and Advance Care Planning, and serve as contacts in their community when any issues related to ACP and POLST arise.”

Membership meetings are the fourth Tuesday January through November at 8:30AM. Meetings are virtual and on a quarterly basis a hybrid meeting is held for in-person networking opportunities and to see your friends in person.
